Requirements MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE) One year of experience in Home Health Care preferred Hospice experience preferred Valid Driver's License, car insurance and reliable transportation Job Duties: Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans Provides care under the direction of a physician.
